It is one of the most common assumptions about hair loss: my scalp is oily, so that must be why my hair is falling out. Wash more, buy the right shampoo, and the shedding should stop. Sometimes that helps a little. Often it misses the real story. The honest answer to "is my hair falling out because my scalp is too oily?" is: not always.
An oily scalp can contribute to shedding. But a clean, well-maintained scalp does not guarantee healthy hair. Hair thinning is frequently a symptom of a deeper imbalance or deficiency, and the most useful clue is what else is happening in your body at the same time.
Can an oily scalp actually cause hair loss?
It can play a role, but it is rarely the whole cause. A very oily scalp can go hand in hand with seborrheic dermatitis, an inflammatory, flaky, itchy scalp condition linked to an overgrowth of a common skin yeast called Malassezia. That inflammation can push some follicles into their resting phase and cause temporary shedding. The key word is temporary. Treat the inflammation and the hair usually recovers.

So one hormonal picture can produce two visible symptoms at once: an oilier scalp and thinner hair. Treating only the oil leaves the other half untouched.
One important nuance for women: you do not always need sky-high hormone levels on a blood test to have this pattern. Many women with pattern hair loss have normal circulating androgen levels and simply have follicles that are more sensitive to them locally (Ho et al., 2023, PMID: 37510231). That is a big reason why guessing from symptoms alone, or from a single lab value, falls short.
The cluster worth paying attention to
A well-maintained scalp does not equal healthy hair, and the biggest clue that your hair loss is hormonal is the company it keeps. Hair thinning becomes much more likely to be hormonal when it shows up alongside other signs of androgen excess or hormonal imbalance:
- Irregular, missing, or unpredictable periods
- Acne, especially along the jaw and chin
- Oily skin and an oily scalp
- Unexplained weight gain, especially around the middle
- Extra hair growth on the face or body
- Bloating and blood sugar swings
